Used correctly, OTC peels are excellent. Used wrong, they can take 6–12 months to recover from.

The peel hierarchy

PHA (gentlest)

Gluconolactone, lactobionic. Larger molecule, slow penetration. Best for sensitive skin or first-time users.

AHA (surface)

Glycolic, lactic, mandelic. Targets pigmentation, dullness, fine lines. Use up to 10% at home.

BHA (oil-soluble)

Salicylic acid. Penetrates oil, ideal for acne and clogged pores. Up to 2% at home.

Combination peels

Many products mix AHA + BHA. Effective but more irritating — start once weekly.

Daily acids

Lower strength acid toners (5–7%) used 3–5x weekly often outperform weekly aggressive peels.

Never at home

TCA >10%, phenol, Jessner's, modified Hetter — these are dermatologist-only procedures.

Choose by concern

Dullness, surface pigmentation, fine lines

Acne, blackheads, oily skin

Salicylic acid 1–2% 2–3x weekly. Penetrates the lipid environment of follicles where comedones form. Pair with retinoid + BPO for complete acne control.

Sensitive, reactive, or rosacea-prone skin

PHA (gluconolactone) or mandelic acid 5% weekly. Both have larger molecules that penetrate slowly, minimising flushing and stinging.

Skin of colour

Mandelic acid or lactic acid 5–10% are safer than glycolic in Fitzpatrick IV–VI because they reduce post-inflammatory hyperpigmentation risk. Avoid TCA and phenol entirely without dermatologist supervision.

At-home peel protocol

  • Patch test on inner forearm for 3 nights before face use
  • Apply to clean, dry skin in evening only — never AM (photosensitivity)
  • Avoid eye area, lips, and any active breakout or wound
  • Start with shortest contact time (1–3 minutes if rinse-off)
  • Build to recommended frequency over 4 weeks
  • Daily SPF 30+ — non-negotiable when using any acid

When to combine, when to alternate

Same routine OK: PHA + niacinamide; lactic + hyaluronic acid; salicylic + benzoyl peroxide (for acne).

Alternate nights: Any AHA/BHA with retinoids. Stacking these triples irritation with no extra benefit.

Never combine: Multiple chemical peels in same routine; peels with strong vitamin C (L-ascorbic at 15%+); peels within 5 days of professional treatment.

Stop and see a dermatologist if:

  • White frosting, blistering, or weeping during/after peel — this is a chemical burn
  • Persistent burning >30 seconds after rinsing
  • Dark patches developing 2–4 weeks post-peel (post-inflammatory hyperpigmentation)
  • Sustained redness, swelling, or skin shedding lasting >5 days
  • Any peel intended to "remove" a mole or skin lesion — never do this

When to upgrade to professional peels

At-home peels are a maintenance tool. Real transformation — for melasma, deep acne scarring, significant photoageing — requires professional TCA peels (15–35%), Jessner's, or laser resurfacing. Home use of these agents causes the worst dermatology emergencies I see. Save for in-clinic.

Weekly is plenty for most people. Daily low-strength acids (toner pads) are gentler and often more effective long-term than aggressive weekly peels.

Lactic is gentler and more hydrating; glycolic penetrates deeper and works faster on pigmentation. Mandelic acid is the gentlest option for sensitive or darker skin.
